Position Title: CSU Discharge Planner, FT

Minimum Entry Salary *: Negotiable Based on Experience

Location: Warner Robins, GA

Services Area/Programs: CSU

Description of Duties: The individual in this role will serve as the discharge planner for the crisis stabilization unit. The focus of this program is to provide inpatient, crisis services that support individuals experiencing a mental health and/or substance abuse crisis. The CSU is utilized for individuals undergoing an acute crisis that presents a substantial risk of imminent harm to self or others. The goal is to stabilize the individual through nursing and behavioral supports on a time-limited basis. The role of the Discharge Planner is to provide mental health and substance abuse counseling to all individuals served in the CSU. Duties include developing Crisis Safety Plans, conducting Suicide Risk Assessments, and conducting groups with clients on the unit. This position is responsible for coordinating follow up care with each individual served to ensure a safe and therapeutic transition of care. The Discharge Planner acts as a member the Interdisciplinary Team to ensure continuity of care.

Minimum Qualifications: Bachelor s degree in Human Services or related field. Valid, unrestricted GA Driver s License. Must be computer literate.

Preferred Qualifications: 2+ years experience in the Behavioral Health Field
